Hunter S. Thompson Fear & Loathing is an urban trail ultramarathon held in San Francisco on December 6, 2026. The event uses a single-loop route that begins and ends at Twin Peaks, a high city vista with broad views over San Francisco. The course passes many well-known city neighborhoods and landmarks and emphasizes navigation and self-reliance.

The event is organized as a fat ass format with the stated ethos "No Fees, No Awards, No Aid, No Wimps." Organizers note that aid stations are not guaranteed but are mostly provided from parked vehicles at approximate mile markers. Typical aid locations listed include Twin Peaks (start/finish), Cesar Chavez / 3rd Street, Japantown (Geary & Webster), Coit Tower (Grant & Lombard), Marina Green, Seacliff / 30th Avenue, Lake Merced, Sunset Boulevard, and Stanyan Street.

A cutoff is applied at mile 25 for 50 mile entrants; runners who meet the cutoff continue on the 50 mile route, while others are directed to the 50K route. Since 2024, 50 mile runners are required to run an additional 0.67 mile hourglass loop around Twin Peaks on return in order to reach the full 50 mile distance.
