The 3 Legged Monster Race is a two-person relay on the Ozark Trail in the Mark Twain National Forest. Teams complete three sequential legs: trail running, mountain biking and paddling. The event is offered in two course lengths.

The running leg travels from Ozark Outdoors toward Bass' River Resort through the Huzzah Conservation Area and Mark Twain National Forest, featuring bluff overlooks, mossy rock outcrops, seasonal waterfalls and hardwood forest. The mountain biking leg follows sections of singletrack and gravel, including parts of the Berryman Trail and FR2265, with climbs through oak and pine stands. The paddling leg covers Courtois Creek, Huzzah Creek and the Meramec River, ending at Ozark Outdoors.
There are two base locations: Ozark Outdoors Riverfront Resort for runners and Bass' River Resort for mountain bikers. Aid stations appear along each leg at specified miles and offer water and on-site medical assistance. Paddles are provided by Bass' River Resort and participants are asked not to bring their own. Final mountain bike routing and exact distances may vary. Proceeds support the Ozark Trail Association, a 501c(3) organization. Organizers note limited cell service in the area, no refunds for registration fees, and that waterways may be crowded during the paddle leg.
