The course is a 3.75-mile loop around Cooper River Park. The loop is fully paved and includes about 100 m of rubber track. The route passes the lake and has views of the Philadelphia skyline. The event provides designated personal pit areas and a fully stocked aid station. The loop passes a Wawa on course.

Runners count only full laps; partial laps are not counted. No new laps may be started within 30 minutes of the race end, and all laps must be completed by the race finish. The 100 Miler uses a 30-hour cutoff. To continue past the 24-hour point, 100 Mile entrants must complete 21 laps (78.75 miles); a finisher must complete 27 full laps and cross the finish for the 27th lap. Relay teams consist of four members, with a maximum of one runner on the course at a time; teams may run their final lap together as the only lap allowed to be run together.
