The Great Springs Trail Race at Natural Bridge Caverns offers a variety of off-road distances through the Comal Caverns network. Standard event participants begin their run inside the caverns before transitioning to a surface course that features wooded sections and rolling hills. The 4.1667-mile core loop is used for specific distance challenges and endurance formats.

The Last Person Standing format operates on an hourly schedule. Runners must complete a 4.1667-mile loop, and ruckers a 2.47-mile loop, every 60 minutes to continue. Participants in the ruck division must carry a weighted pack: 20 lbs for those under 150 lbs, and 30 lbs for those over 150 lbs. The competition continues until only one person remains who can finish the loop within the one-hour limit.
The event includes full aid station support at the race hub with water, electrolyte drinks, and various snacks. All races are chip-timed, and the 4, 8, and 16-mile distance rankings are based on chip time. While the venue does not provide overnight camping for standard racers, the race hub serves as a central area for spectators and participants.
