Machete Muertos 200 is part of a multi-day trail running event held at Lake Perris. The event offers multiple distances and a ruck division:

The 200-mile course is reported to include over 44,000 feet of elevation gain and technical terrain. The event uses two primary course formats: a 10 mile plus loop with about 2,300 feet of elevation gain per lap for the 10 and 50 mile races, and a 20 mile loop around the lake with about 4,500 feet of gain used for the 20 mile ruck, 100, and 200 mile distances.

Cutoff times are specified for each distance: 96 hours for the 200 mile race, 48 hours for the 100 mile, 24 hours for the 50 mile, 12 hours for the 20 mile ruck, and no cutoff for the 10 mile.

Aid stations and a transition area are provided; a full aid station is located at mile 6 for the 10 and 50 mile courses, and aid stations for the longer loops are at miles 8 and 13. Pacers are allowed under defined rules and weights for the ruck division are required: 20 pounds for females and 30 pounds for males.
